Compassionate Medical Social Workers (CSWA/ LMSW)

Schedule: Monday – Friday 8:00AM – 5:00PM

Brief Description:
We are looking for a compassionate and motivated licensed social worker who will provide a tender supportive care experience to the patients and families during their visits. As a locally-owned hospice agency, we aim to offer a hospice service done the right way with our tender hearts. Come and join our team!

Job Description:
Assisting patients and families navigate during their most challenging moments is a job not everyone can do. Medical Social Workers (CSWA/ LMSW) serve an exceptionally critical function during those moments that are essential to our team. Be empowered by joining a team of hospice clinicians making a difference to patients.

Job Summary:
Provides onsite psychosocial assessment and plan of care – includes emotional, social, financial, and environmental resources and identifies appropriate psychosocial problems, interventions, and goals for the patient’s plan of care. Offers support to patients and guides them toward comfortable end-of-life care.

Qualifications:

  • MUST have a license (CSWA, LMSW) or are in the process of getting licensed and a master of Social Work degree from a school of social work. New graduates are welcome to our team!
  • Must have excellent problem-solving, critical thinking, and social skills and the ability to build trusting relationships not only with the patient but with all the care team members involved in the care
  • Should be skillful in organization and in the principles of time management and have knowledge of basic nursing skills
  • The ability to communicate well, both verbally and in writing.
  • Must have the ability to demonstrate self-confidence and a positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
  • Must be able to fulfill all training requirements per the Hospice’s policy.
  • Must be able to perform roles within the safety standards per the Hospice’s Policy and Procedure and licensing standards.
  • Must be able to represent Enlight Hospice of VA, Lighthearted Principle, Mission and Vision.

Compassionate RN Case Manager

Schedule: Monday – Friday 8:00AM – 5:00PM

Brief Description:
We are seeking for a full-time compassionate and motivated RN Case Manager who will provide a tender supportive care experience to the patients and families.

Job Summary:
Lead in planning, coordination and provision of hospice care in accordance with the patient’s plan of care. Provides excellent bedside manners and care to achieve maximum comfort of patient during end of life care.

Qualifications:

  • Must be a Registered Nurse licensed by the state. A minimum of 1 year experience in home health, skilled nursing facility, memory care unit or hospice, preferred.
    New nurses welcome – we have a great mentorship group!
  • Must have an excellent problem-solving, critical thinking and social skills
  • Must have the ability to extend a holistic care experience to achieve maximum comfort and dignity during the patient’s end of life journey.
  • Ability build trusting relationships not only with the patient but with all the care team members involved in care
