- Aged 65 and older
- Experienced multiple falls and emergency room visits within the past 6 months
- Noticeable decline in overall health and functional abilities
- Presenting symptoms include weight loss, decreased appetite, and various physical, emotional, and behavioral challenges stemming from their diagnosis
